PHYSICIAN REFERRAL FORM

Step 1: Complete a FREE ADHD Self Report Scale

A full assessment for ADHD is more than simply completing a scale, but you might be curious to see if you have symptoms of ADHD. If you complete this scale and notice you have more than 4 of the symptoms, you may want to request a referral from your provider (doctor or NP) to our physicians for an MSP-covered ADHD assessment.

Step 2: Get Referred

 

Referral for assessment on pause until 2025. 

We hope our assessment process will leave you feeling educated, clearer about your own diagnosis, and empowered to use this new knowledge to start creating a plan for utilizing your strengths and overcoming your challenges to Level Up your life. You can request that a Doctor or Nurse Practitioner completes the referral form and sends it to us.

Step 3: Complete Intake Forms and Get Assessed

In order to be diagnosed and receive support for ADHD you need an assessment by a trained healthcare provider. This can be done by a trained physician, nurse practitioner, or psychologist. We offer MSP-covered ADHD assessment with our physicians by referral only.

Once we receive your referral you will hear from her office in approximately 4 weeks with information about wait times and some intake forms to complete.

After the intake forms are completed you will be given the options to book your appointment. All appointments are on Wednesday mornings or Thursday afternoons. Typically an ADHD assessment will take between 2-3 appointments.

During your assessment, we’ll review your intake forms, ask you questions about your medical history and your symptoms, and answer any questions you might have about ADHD.

If you are diagnosed with ADHD and would like to start on medications, we can initiate medication, and follow you for up to 3 months before discharging you back to your regular provider; or, if you or your provider prefers, we can discharge you back to your provider with recommendations for medications to start.

 

Step 4: Review Results with Our Physician

 

Once we have completed the assessment process with you over 2-3 appointments we will review your results and make recommendations for treatment.

Step 5: Consider Treatment, Join our Community, and Get Support

 

There is a lot of information out there, and it can be difficult to wade through it all yourself. ADHD is a spectrum, with different symptoms, different degrees of severity, and different types of issues depending on the circumstances. Finding the right medication and lifestyle interventions that will help you be successful can involve some trial and error. Level Up ADHD provides a holistic approach to improving your ADHD game.
